What happened
Transformer Magazine reported that a transformer explosion in Toronto, Canada, caused a power surge that sparked fires and knocked out electricity to some homes. Firefighters reported arriving within minutes and found very thick black smoke billowing from the back of a home about four doors down. The report says electricity has since been restored and that residents who lost power were advised to keep their circuit breakers off until their electrical systems could be inspected.
The Toronto Premium Outlets mall was evacuated as a precaution after reports of smoke. Police said all occupants got out safely, and normal operations have since resumed. The report does not give the exact date of the incident, the cause of the explosion, the owner or operator of the transformer, or the duration of the outage, and does not say whether the transformer was at or near the mall.
Contributing factors
The source does not state the cause of the explosion. It does not say whether the transformer was located at or near the mall or the affected home.
Lessons learned
- Transformer failures can cause fires in nearby buildings, so emergency plans for substations and pole-mounted equipment should cover adjacent properties.
- Inspect household and building wiring before circuits are re-energised after a surge or fault.
- Precautionary evacuation of nearby commercial premises during smoke events helps protect occupants while the cause is assessed.
More lessons on spacing, layout and fire spread, maintenance, inspection and ageing assets, emergency response and public communication, from every incident on the register.
