What happened
On 26 March 2022 a fault occurred in part of the power supply system at a data centre operated by Kyndryl Japan. The fault began at 11:08 am, and power was restored at 11:44 am the same day, so the power disruption lasted about 36 minutes. The systems of some customers using the data centre were affected. Kyndryl Japan and IBM Japan announced the failure on 28 March 2022. The source does not give the location of the data centre, the equipment that failed, or the cause of the fault.
Contributing factors
Not reported. The source does not state the cause of the fault in the part of the power supply system that failed.
Lessons learned
- Test and monitor redundant power paths so that a fault in one part of the supply does not interrupt customer systems.
- Set out in advance how customers will be notified of a power failure and how quickly, so that notice can be given without delay.
- Record the fault sequence and confirmed root cause so the operator can establish whether the same failure could recur.
