What happened
In the early morning of 11 September 2026 a lineman employed by a subcontracting company was seriously injured while carrying out electrical repair work at the Lakeview Avenue electric substation in Mayville, New York. The substation is owned by the Village of Mayville. The work took place during a scheduled villagewide power outage that began at midnight and lasted a few hours. Sheriff's deputies arrived, administered CPR and used an automated external defibrillator before handing the casualty to emergency medical services.
He was taken to hospital and later died, on 15 September 2026, according to his obituary as reported by the source. A lawyer for the family said the investigation into the circumstances is ongoing. The cause had not been reported at the time of publication.
Contributing factors
Not reported. The same substation had suffered failed mini-transformers and a small fire on 2 September 2026 (EIR-0626), and the village declared a state of emergency on 8 September to speed repairs. No link between those events and the fatal injury has been established.
Lessons learned
- Treat emergency repair work on substations with particular care, and confirm isolation and earthing before work starts, even during a planned outage.
- Where a site has recently failed, plan any follow-on work again from the start, because pressure to restore supply quickly can compress planning and safety checks.
- Brief subcontractors on site-specific equipment and emergency arrangements, and rehearse first aid and defibrillator response with local emergency services.
