Lessons learned
- A transformer fire at a distribution substation can cut power to thousands of customers even when no injuries are reported.
- Where the cause is not known after the initial response, operators should preserve evidence and publish findings when they are established.
- Restoration planning should assume a large emergency-service attendance and a need to re-energise customers without knowing the failure mode on the day.
